Disease management is a series
of planned interventions to help an individual better manage
a chronic disease. Education and coaching are provided on
symptoms, medications, side effects, nutrition, exercise
and other self-care behaviors to adhere to the prescribed
care plan and improve the patient’s quality of life.
Designed to the specific needs of each patient, the Total
Access Medical personal care program incorporates and coordinates
all vital elements of individual care, including education
and self-management tools, psychosocial support, case and
disease management, recommendations of appropriate prescription
medications, as well as coordination with specialty physicians.
Through proactive management and monitoring of all critical
aspects of patient care, TAM physicians help patients control
their symptoms, stay on track with medications and lead healthier
lives. |
